Your medical plan options

Essential HSA

You’ll pay the least out of your paycheck, though this plan doesn’t pay for services (other than in-network preventive care) until you reach the out-of-pocket maximum. If you pay for eligible services that exceed the out-of-pocket maximum ($5,000 individual / $10,000 family), the plan will pay for all eligible services for the rest of the calendar year.

Select HSA

You’ll pay less out of your paycheck than for the PPO, and you’ll get free money from the Company contributed to a tax-advantaged Health Savings Account (HSA).

Premium PPO

You’ll pay more out of your paycheck in exchange for a lower deductible and out-of-pocket maximum.

HSA Plans

These plans give you the opportunity to use a tax-advantaged Health Savings Account (HSA). Heritage will make a contribution to the HSA to start your savings. You can contribute to the account tax-free, use it to pay for eligible health care expenses, and let it grow – the account is yours to take with you if you leave the Company, and you can use it for health care expenses in retirement.

With the HSA Plans, there are no copays for prescription drugs. After you meet the deductible, you'll pay a percentage of the cost of your prescription. Watch the video to learn more.

Premium PPO Plan

The PPO plan offers lower out-of-pocket costs in exchange for higher employee contributions. With this plan, your costs are more predictable, but you’ll still have out-of-pocket expenses for some services, and the plan reimburses a percentage of the cost after the deductible is met. Watch the video to learn more.

Supplemental Medical Overview

We know life doesn’t always go as expected, which is why Heritage provides access to additional programs designed to protect you and your family from the financial impact of a significant unexpected medical expense due to illness, accident or hospitalization. Heritage offers the following supplemental medical coverage options through Voya. See Employee Contributions for details on what coverage costs.

Accident insurance

Accident insurance helps protect from unexpected financial stress if you or a covered family member has an accident. Benefits are paid based on the injury and the treatment needed, including emergency room care and related surgery. The benefit can help offset the out-of-pocket expenses that medical insurance may not cover, including deductibles and copays. Coverage is available for you, your spouse, and dependent children. Watch the video to learn more.

The actual benefit amount depends on the type of injuries and the medical services needed. See Employee Contributions for details on what coverage costs.

Critical illness insurance

If you or your dependent is diagnosed with a serious illness, critical illness insurance can provide financial support to help you through a difficult time. This benefit protects against the financial impact of certain illnesses, such as:

  • Heart attack
  • Stroke
  • Cancer
  • Major organ transplant
  • Alzheimer’s disease

If you experience a covered illness, you’ll receive a lump-sum payment to use at your discretion for expenses such as coinsurance, deductibles, travel to treatment centers, experimental treatments, and childcare.

The following coverage options are available:

  • Employee coverage: Choose $10,000 or $20,000
  • Spouse coverage: 50% of employee benefit amount
  • Child coverage: 50% of employee benefit amount

See Employee Contributions for details on what coverage costs. Watch the video to learn more.

Hospital indemnity insurance

A trip to the hospital can be stressful, and so can the bills. Hospital indemnity insurance provides benefits to help alleviate the unexpected costs of a hospital stay. The plan pays benefits to help cover expenses — such as deductibles, coinsurance, or household bills — when admitted to the hospital for a covered illness or injury. Coverage is available for you, your spouse, and dependent children.

The following coverage options are available:

  • Option 1: $1,000 benefit for the first day, and $100 daily benefit beginning day 2, for up to 10 days
  • Option 2: $2,000 benefit for the first day, and $200 daily benefit beginning day 2, for up to 10 days
